Should there be a in-text citation after every sentence when the same source is pharased multiple times?
No, this can be visually jarring to the reader. A general advice is to place the in-text citation at the beginning of the paragraph. If necessary, the author's name can be repeated and/or include the name at the end of the paragraph:

Johnson also says that …
